Job Role Summary
The Sandra Eskenazi Mental Health Center Employment Consultant provides assistance to individuals with physical and/or psychiatric disabilities in obtaining and retaining competitive work in a community setting. This position is responsible for orienting, training, coaching and supporting individuals to make placements successful.
Essential Functions and Responsibilities
Provides assistance to individuals with physical and/or psychiatric disabilities in obtaining and retaining competitive work in a community setting
Responsible for orienting, training, coaching and supporting individuals to make placements successful
Completes intake and assessment to determine optimal employment opportunity for each individual, including assessment for vocational interest, support systems, past/present problem areas, assets/potentials, skill and needs
Formulates a vocational rehabilitation plan in collaboration with the individual and their primary Clinician/Care Coordinators and others as requested by the individual.
Participates in interdisciplinary meetings and scheduled conferences providing appropriate input
Carries out job matching/development through comprehensive search in community for potential employment sources which includes making contacts with potential employers and developing ongoing working relationship with them
Develops job specifically to match patient needs
Completes all required client/service activity forms to ensure accurate information for client billing, setting fees and management information according to the Center policies and procedures
Completes FSSA documentation and record keeping as required
Meets the agency standards of timely completion of all documentation
Meets the agency standards of service hours
Job Requirements
Bachelor's degree required, with the ability to meet minimum competency standards set forth by Eskenazi Health Sandra Eskenazi Mental Health Center CMH
Valid Indiana Driver's License
Knowledge, Skills & Abilities
Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data/interpret appropriate information needed to identify each patient's requirements relative to age specific needs
Knowledge of the process and parameter of severe mental illness, problems and potentials of patients, be patient centered, committed to principles of support employment
Recognize people's inherent right to work and to make choices concerning their work
Establish contacts with regard to Sandra Eskenazi Mental Health Center /Community resources, information, forms that are needed
Familiar with business sector protocol and procedures, public relations, and marketing skills that can be applied to assist individuals with disabilities
Knowledge of the process and philosophy of supportive employment
Verbal and written communication skills needed
Ability to:
Use age appropriate communication skills
Use variety of reality oriented problem solving counseling and assessment techniques
Communicate verbally and in writing in a clear thorough/concise manner (writÂing must be legible)
