The Opportunity
Working closely with the Clinical Informatics Program Manager and the Program Director for Digital Health Systems Operations, the Population Health Management (PHM) Project Manager is responsible for activities as assigned, related to leveraging the enterprise electronic health record (EHR) to advance the delivery of high quality, safe, value-based patient care across the populations of patients cared for at Mass General Brigham (MGB). This includes project managing the development of tools such as patient registries, reports and best practice alerts, by serving as a conduit between the clinical architects' needs and the staff overseeing the design, build and maintenance of the EHR in designing solutions.
The PHM Project Manager works closely with the enterprise PHM Program to assess opportunities for using the EHR to improve clinical outcomes and works closely with Digital eCare staff to align solutions with desired outcomes and with Digital platform processes for project completion. This includes clinical governance meetings and tracking project(s)' progress.
The PHM Project Manager ensures that all clinical content is appropriately tracked in an inventory database. The Project Manager identifies and mitigates project risks to achieve successful project completion across MGB. The Project Manager participates in eCare staff meetings and separately in PHM meetings, serving as a bridge between the two groups to ensure eCare is appropriately organized to deliver on PHM projects.
General Responsibilities
• Supports PHM in leading the system-wide efforts to leverage Epic as a tool to advance program goals across MGB.
• Leverage knowledge of ambulatory and inpatient care and population health management to benefit the PHM program, and Digital eCare (DeC). This includes all aspects of the management of care across populations including effectiveness, safety, patient-centered, value-based care, equity, efficiency, and timeliness.
• Works to manage project dependencies and timelines across DeC and PHM to reconcile competing priorities.
Mass General Brigham Population Health Responsibilities
• Assist with intake of requests across Population Health and work with PHM Clinical Architect to triage requests
• Facilitate program meetings associated with Digital Technology
• Maintain coordination between PHM needs and Digital Health Systems Operations processes
• Track PHM work efforts though the Digital lifecycle as key point of contact for PHM
Digital eCare Responsibilities
• Supports the development and management of a portfolio of work as the lead Project Manager for Population Health Management.
• In conjunction with the Clinical Informatics Program Manager, coordinates with leadership of relevant Digital teams to achieve PHM goals including Clinical Decision Support, Clinical Content, Process Redesign, Reporting, and Training.
• Manages the process for content development, standardization, coordination, maintenance and archiving.
• Project management coordination responsibilities for clinical informatics across clinical content disciplines, informatics, biomedical device integration and MGB programs.
• Manages and coordinates gathering and synthesis of current clinical content and the development of new clinical content.
• Leads assigned work groups and facilitates interactions and communication to arrive at solutions for content development and management.
• Provides coordination and direction to complete the assigned clinical content tasks, produce the required deliverables, track and resolve issues, and meet the relevant Digital eCare project milestones according to the established timeline.
• Collaborates with the Clinical Informatics Team, and the Clinical Application Project Directors and Application Managers, as well as clinicians and operational leaders from across MGB.
• Manages project issues and identifies risks and mitigation strategies using strong analytical and problem-solving skills.
• Reports project status to the Clinical Informatics Program Manager/ Director's/ CMIO
• Other responsibilities as determined by PHM leadership, the Clinical Informatics Program Manager and the Program Director for Digital Health Systems Operations.
Qualifications
Qualifications
• At least 5 years of project management experience
• Experience with clinical care delivery across health disciplines and the care continuum
• Experience with clinical content development and maintenance
• Experience managing multiple concurrent large projects and project teams
• Experience in meeting management, facilitation and presentation required
• Knowledge of Population Health Management
• Bachelor's degree required, master's degree preferred, in a clinical area of expertise, IT or public health
Skills/Abilities/Competencies Required
