Location: Virtual; Candidate must be within the state of Kentucky
Shift: Monday - Friday; 8:00am - 4:30pm ET with occasional evening availability
The Exercise Physiologist is responsible for providing exercise guidance to disease management members.
How you will make an impact:
Collaborates with nurse case managers in managing disease management health goals of members.
Reinforces the primary care physicians exercise recommendations providing safe and effective exercise instruction and guidance.
Provides member health coaching, education, assessment, and support of behavioral changes to promote self-management of chronic conditions.
Provides members with education materials related specifically to member score condition.
Minimum Requirements:
Requires a BS/BA degree in exercise science, exercise physiology, kinesiology, physical therapy or physical education and a minimum of 3 years experience as an exercise specialist, exercise physiologist or fitness instructor/trainer developing exercise prescriptions for special populations; or any combination of education and experience which would provide an equivalent background.
Preferred skills, capabilities, and experiences:
A minimum of 1-year cardiac rehab, pulmonary rehab, and physical therapy experience in clinical setting preferred.
Certification by the American College of Sports Medicine (ACSM) as a Health Fitness Instructor preferred.
MS in a related field and membership in American Society of Exercise Physiologist preferred.
