Overview of the Role:

Alignment Health is seeking am organized, compassionate, and tech savvy bilingual Spanish telephonic LVN case manager (must have California Licensure) to join the remote case management team. As a case manager, you are responsible for health care management and coordination within the scope of licensure for Alignment Health members with short term chronic care needs, based on the care plans provided by providers. You will implement, coordinate, and monitor the care plans designed to optimize member health care across the care continuum. You will also coordinate and monitor members' progress and services to ensure consistent, cost-effective care that complies with Alignment Health policy and all state and federal regulations and guidelines.

Schedule: Monday through Friday


  • 8:00am - 5:00pm Pacific Time
  • 9:00am - 6:00pm Mountain Time
  • 10:00am - 7:00pm Central Time
  • 11:00am - 8:00pm Eastern Time

Responsibilities:

  • Ensure member access to services appropriate to their health needs.
  • Identify, assess, and manage members per established criteria.
  • Review and implement care plans and provide education materials as necessary with members.
  • Interface with primary care physicians (PCP), nurse practitioners (NP) RN case managers, and specialists for coordination of care for members.
  • Assist in problem solving with providers, RN case managers, and / or service issues.
  • Measure the effectiveness of interventions to determine case management referrals and outcomes.
  • Participate and provide feedback in the development of utilization / care management policies and procedures.
  • Counsel and engages in personal discussions with patients and their families on available care options; help them to determine their appropriate and preferred course of action.
  • Provide follow up and implement care plan based on nurse practitioner.

Required skills and experience:

  • Minimum (2) years' clinical case management experience; or any combination of education and experience, which would provide an equivalent background.
  • Current, unrestricted LVN license in California.
  • Associate's degree preferred
  • Bilingual English and Spanish, Vietnamese, or Chinese highly preferred.

PAY RANGE: $73,200.00-$85,000.00 annually
