Telephonic Behavioral Health Case Manager

Are you ready to make a significant impact in the lives of individuals battling behavioral health and substance abuse issues? Our team is seeking an enthusiastic and dedicated Behavioral Health Case Manager to join us in a fully telephonic role.

What You'll Do:



  • Telephonic Case Management: Utilize your expertise to manage cases and provide support to members with behavioral health and substance abuse or substance abuse disorder needs.


  • Comprehensive Assessments: Conduct thorough assessments to identify individual needs and develop personalized care plans that address objectives and goals.


  • Continuous Evaluation: Monitor and evaluate the effectiveness of care plans, modifying them as necessary to ensure optimal outcomes.


  • Resource Coordination: Collaborate with internal and external resources to meet the identified needs of members, ensuring they receive appropriate, quality, and cost-effective care.


  • Provider Collaboration: Work closely with providers to coordinate care and support member access to necessary services.


What You'll Bring:


  • Educational Background: Requires a MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing.


  • Clinical Experience: A minimum of 3 years clinical experience in social work or counseling, with a broad range of experience in treating complex psychiatric and substance abuse or substance abuse disorder treatment.


  • Licensure: Must hold a current active, unrestricted license such as LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as allowed by state laws and scope of practice.


  • Preferred Experience: Case management and telephonic coaching experience with members dealing with a broad range of complex psychiatric, substance abuse, and/or medical disorders.


Additional Requirements:

  • For Government business: LAPC, LAMFT (as allowed by applicable state laws), and other relevant state or federal licensure.

  • For associates working in Puerto Rico: A current PR health certificate and a current PR Law 300 certificate are required for this position.
