The Physical Therapist evaluates, plans, organizes, and participates in rehabilitation programs to improve mobility, relieve pain, increase strength, and prevent or reduce deformity in patients with diseases or injuries. Services are provided throughout the care continuum in a cost-effective, quality-focused, and customer-oriented manner while adhering to all regulatory standards.
Core Responsibilities and Essential Functions:
Patient Assessment:
Collaborate with physicians to develop and evaluate treatment programs, care plans, goals, and discharge plans based on the patient's physical therapy needs.
Create and adjust a plan of care with input from patients and/or family members.
Develop realistic discharge plans incorporating patient and/or family goals.
Treatment Planning and Provision of Care:
Implement the physical therapy treatment plan.
Select appropriate activities to help patients achieve their goals.
Revise goals and plans of care with input from patients and families.
Provide ongoing education and training to patients and families.
Offer discharge instructions and referrals to community resources as needed.
Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
Documentation:
Document evaluations and care plans according to departmental guidelines.
Record each treatment session following departmental protocols.
Complete and submit required documentation, including family education and discharge information, in accordance with facility and program guidelines.

Required Minimum Education:
Graduate from an accredited physical therapy program.
Bachelor's, Master's, or Doctorate in Physical Therapy required.
