Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Centeroffers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary




The CCBHC Acute Care/ED Liaison ensures continuity of care for Certified Community Behavioral Health Clinic (CCBHC) clients between the Sandra Eskenazi Mental Health Center CCBHC and acute care or emergency rooms in designated CCBHC service area. This role focuses on ensuring safe and healthy transitions of care between local hospital emergency departments, acute psychiatric emergency departments, psychiatric triages, and crisis receiving and stabilization facilities and the CCBHC. The Liaison role develops and maintains strong collaborations with those healthcare providers previously mentioned. The Liaison ensures that the CCBHC client is connected to their primary team with the minimal time needed to do so between discharge and treatment follow-up, consistent with CCBHC standards. The Liaison maintains close collaboration with the CCBHC outpatient treatment teams and continues to actively work with the CCBHC client in the community until the primary CCBHC treatment team can fully engage the client. The Liaison additionally has the important role of tracking and reporting their work with CCBHC clients consistent with CCBHC quality metrics. The Liaison also works closely with both Sandra Eskenazi Mental Health Center Mental Health Recovery Center and CCBHC outpatient treatment teams to file, track, report, and assist in maintenance of all forms of mental health commitments.




Essential Functions and Responsibilities






  • Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing

  • Routinely works with local hospital emergency departments, including acute psychiatric emergency departments, psychiatric triages, and crisis receiving and stabilization facilities to ensure valid and timely communication at time of CCBHC client admission

  • Coordinates and helps CCBHC clients navigate post-discharge transition to outpatient or other needed treatment to minimize time between discharge and treatment follow-up

  • Assists in advocating for CCBHC clients to ensure access to needed services which includes developing community relationships and resources

  • Develop, revises, or monitors crisis prevention and safety plans to reflect current CCBHC client status and needs

  • Monitors and documents CCBHC client progress including reasonable attempts to contact CCBHC clients post-discharge

  • Actively tracks CCBHC client admissions to local hospital emergency departments, including acute psychiatric emergency departments, psychiatric triages, and crisis receiving and stabilization facilities

  • Actively works with CCBHC outpatient treatment teams to coordinate transition of CCBHC client to community living post-acute care discharge, including attending relevant team staffings and participation in the Acute Services morning call

  • Provides case management including working with family, significant others and community supports to assist CCBHC clients transition between acute care and outpatient settings

  • Meets Sandra Eskenazi Mental Health Center requirements for continuing education in accordance with best practice, CCBHC, Joint Commission, and other regulatory standards

  • May provide transportation in company provided vehicle and/or personal vehicle as directed by treatment team and guided by medical necessity; maintains a copy of Eskenazi Health's vehicle-related accident reporting procedures in vehicle






Job Requirements






  • Bachelor's degree in human services, psychology, social work, or a related field with two years of relevant work experience required; Master's degree in human services psychology, social work, or a related field preferred

  • Valid Indiana behavioral health and human services (BHHS) license in good standing that meets Indiana BHHS licensure requirements is required, dependent upon the degree conferred (i.e. bachelor's in social work must have valid Indiana Licensed Bachelor Social Worker license, etc.)

  • Valid Indiana driver's license and driving record that meets Eskenazi Health's driving policy requirement

  • Basic knowledge of CCBHC standards regarding transitions of care

  • Knowledge of the Sandra Eskenazi Mental Health Center crisis continuum, outpatient services, and acute care services.

  • Basic knowledge of Marion County's crisis continuum

  • Basic knowledge of Marion County's mental health, substance use, and hospital providers

  • Basic knowledge of substance use disorder and/or co-occurring disorders

  • Knowledge of client rights and responsibilities

  • Basic knowledge of psychotropic medication, their uses and side effects

  • Must meet all population-specific competencies for role and service area

  • Knowledge of trauma-formed, person-centered, family-centered, and recovery-oriented care.
