DHHS, Division of Children and Family Services has an exciting new program - Therapeutic Family Care (TFC). This is a program that will provide clinical care coordination to high tiers in foster care and clinical crisis services, with the outcome of serving the youth in community-based care to prevent a more restrictive institutional placement. Work is completed in a non-institutional setting.

Examples of work



Under limited supervision, the CMSW:


  • Performs Licensed Certified Master's level professional social work in determining patient needs and providing complex therapeutic recommendations.

  • Completes behavioral health assessment and determination for TFC program eligibility.

  • Leads a care coordination team for each eligible youth.

  • Serves as a member on interdisciplinary teams to participate in care plans for assigned youth, as applicable; provides clinical guidance to social workers, direct staff, practicum students, and/or volunteers.

  • Work is performed under the supervision of a CMSW Supervisor, Nurse Administrator, or Administrator. Performs related work as assigned.



For clinical programs with an assigned caseload:


  • Provides clinical care coordination consultation to social work and social work support staff.

  • Conducts comprehensive behavioral health assessments on an assigned caseload, as applicable.

  • Conducts individual, family, and provider team meetings to assist clients and families in plan of care, as applicable.

  • Compiles social and behavioral information from various sources. Serves as a member of an interdisciplinary team; provides information regarding a client's social history and psychosocial assessments and addressing specific client needs; participates in the development of a client's active plan of care.

  • Monitors and evaluates patients' progress toward established goals of discharge; provides recommendations to members of an interdisciplinary treatment team to adjust a client's treatment plan.



For clinical programs with quality oversight:


  • Conduct research of medical records, case files, reports, journals, and other pertinent documentation to verify compliance with federal regulations.

  • Serves as liaison with service agencies consulting on member cases to ensure efficiency and continuity in providing services.

  • Conducts follow-up assessments and evaluations and aids in resource development and in policy and program development and evaluation.




Requirements / Qualifications




Minimum Qualifications:


Certified Master Social Worker- Master's degree in social work and two years of Master Social Worker experience.


Master Social Worker- Master's degree in social work.



Preferred Qualifications: Any combination of training and/or work experience that will enable the incumbent to possess the required knowledge, skills and abilities. Positions allocated to the Certified Master Social Worker class for clinical programs may perform work in this class with a provisional certification. A current license to practice as a Social Worker in Nebraska and one year of relevant experience, as applicable for clinical program with assigned patients; current certification as a Masters Social Worker in Nebraska and one year of relevant experience, as applicable for clinical programs with quality oversight.



Hiring rate: Master Social Worker - $26.969


Certified Master Social Worker - $28.990



Hours: M-F 8AM-5PM



Knowledge, Skills, Abilities



Knowledge of: theories and methods of psychosocial assessment; social systems, family systems, systems theory, and social institutions; the cause of social problem areas and their impact on individuals, families, and communities, as applicable; State and federal laws and regulations affecting social, legal, and health services; crisis intervention theories and techniques; professional terminology as related to therapy, psychology, and human services.


Skill in: observing and interpreting verbal and non-verbal behavior; interviewing clients to collect and elicit essential information and assess needs and progress, as applicable; listening to and understanding information received from others; communicating so others understand; conducting differential diagnosis of individuals and groups; utilizing personality theory and diagnostic methods in setting a therapeutic context and formulating goals; assessing functional impairments so they can be addressed in treatment and discharge planning; assembling relevant facts to formulate a comprehensive psychosocial evaluation; discussing sensitive/emotional subjects in a non-threatening, supportive manner; utilizing appropriate social work intervention methods to build client strengths and facilitate positive change; establishing and maintaining rapport with clients and families.


Ability to: serve as a role model in a professional relationship with patients, members, and social work staff; communicate social work practice and theory to professionals in other disciplines; mediate negotiations between patients, families/guardians and clinicians, as applicable; generate creative and effective means of dealing with obstacles in the implementation of discharge plans and the provision of social work services to clients; write concise and accurate reports and correspondence.
