Key Responsibilities:
Provide physical therapy services to residents in a skilled nursing facility setting
Create individualized treatment plans to meet the needs and goals of each patient
Collaborate with an interdisciplinary team to ensure comprehensive patient care
Maintain accurate documentation and patient progress reports
What We're Looking For:
Licensed Physical Therapist (PT) in the state of California
Strong communication and teamwork skills
Compassionate, patient-focused, and detail-oriented
Experience in a skilled nursing or rehabilitation facility is preferred but not required
