The Opportunity
Collaborate with the multidisciplinary healthcare teams to develop comprehensive care plans tailored to individual patient needs and preferences.
Collaboration in interdisciplinary meetings to discuss patient progress, treatment plans, and discharge planning.
Conduct routine rounds to assess patient status, identify emerging issues, and address immediate care needs.
Collaborate with physicians, specialists, and other healthcare providers regarding the plan of care with a focus on discharge planning.
Actively participate in plan of care meetings to facilitate communication amongst the patient care multidisciplinary team.
Act as a liaison between the provider group and other members of the health care team with a focus on Case Management and nursing allowing the provider to function at the highest level of the scope of their role.
Collaborate with members of the patient care team in the patient discharge planning process for safe discharge planning and optimal patient outcome.
Assist in tracking the provider team's metric performance to support contract retention and incentive metric capture.
Assist in identifying and mitigating barriers to discharge by working with all members of the multidisciplinary care team.
Facilitate communication as a point of contact for the provider team with other disciplines.
Foster effective communication and collaboration among healthcare team members to promote seamless care delivery.
Serve as a resource and mentor for nursing staff, providing guidance on best practices in patient care coordination and rounding.
Communicate with patients and families in a clear, empathetic manner, addressing questions, concerns, and preferences sensitively.
Collaborate with case managers, social workers, and community resources to facilitate post-discharge care transitions and continuity of care.
Participate in quality improvement initiatives and interdisciplinary committees to enhance patient care processes and outcomes.
Promote and maintain the mission, values, and culture of Vituity.
Provide leadership in assessing common or overlapping needs and identify opportunities for consolidation or optimization in delivery.
Advise colleagues and leadership in navigating and utilizing Vituity resources to implement products and services as needed.
Provide leadership, planning, implementation, and management of performance and quality improvement programs that help assess and improve operational performance, contract stability, patient satisfaction, financial viability, and knowledge dissemination for the practice line.
Collaborate with leadership on project objectives and deliverables.
Support incentive structures related to finance and quality.
Promote and maintain the mission, values, and culture of Vituity.
Required Experience and Competencies
Current licensure as a Registered Nurse in the state of practice required.
Minimum of 2-3 years of clinical nursing experience in a healthcare setting, with demonstrated proficiency in care coordination and direct patient care required.
Comprehensive understanding of nursing principles, procedures, and practices relevant to patient care coordination.
Proficiency in assessing patient needs, developing care plans, and implementing interventions to promote optimal health outcomes.
Strong organizational skills and attention to detail, with the ability to prioritize tasks and manage multiple responsibilities effectively.
Excellent interpersonal and communication skills, with the ability to interact professionally with patients, families, and healthcare team members.
Proficiency in electronic health record systems and other healthcare technology platforms.
Commitment to continuous learning and professional development in the field of nursing and care coordination.
Leadership qualities to inspire and motivate team members, foster a collaborative work environment, and promote a culture of excellence in patient care coordination.
Ability to work collaboratively with healthcare professionals from diverse backgrounds and disciplines, demonstrating respect, professionalism, and cultural competence.
Ability to effectively collaborate with interdisciplinary teams, including physicians, therapists, social workers, and case managers, to coordinate and manage patient care transitions.
Knowledge of community resources and support services available to patients and families to facilitate continuity of care and promote holistic well-being.
Ability to use diverse organizational toolsets to recognize root causes at sites and pull needed resources to resolve problems.
Ability to have high level communication with site leaders on all levels.

Wynn Hospital - Utica, New York
State-of-the-art, 373-bed facility with private patient rooms, behavioral health ED treatment rooms, a birthing center, and a helistop.
Level III Trauma Center and a Comprehensive Stroke Center Program.
Services offered include surgery, ICU, pediatrics, psychiatry, oncology, cardiology, wound care, laboratory and imaging services, and more.

We are excited to share the base salary range for this position is $33.05 - $41.31, exclusive of fringe benefits or potential bonuses. This position is also eligible to participate in our annual corporate Success Sharing bonus program, which is based on the company's annual performance. If you are hired at Vituity, your final base salary compensation will be determined based on factors such as skills, education, and/or experience.
