Summary:
Referral coordinator facilitates retrieval of all medical and diagnostic reports and images for new referrals; verifies the accuracy of information obtained and schedules all new patient consults/evaluations.
Responsibilities:
Contacts patients and/or providers to obtain all pertinent medical records and verifies the accuracy of all information obtained.
Participate in special projects or activities as requested.
Download Compact Discs (CD) through the PACS scanning system and transfers images to patient chart.
Work closely with nurse navigators, APRN's and physicians to ensure all patient information and documentation is obtained.

Education and Certification Requirements:
High School Diploma or Equivalent (Required)

Complexity of Work: Knowledge of Microsoft word and excel. Excellent communication skills, reliable, self-motivated and competent. Required Work Experience: Three (3) years of experience in a related healthcare field.
