Summary:
As the Integrated Care Nurse Supervisor, you will oversee the provision of medical and preventative support services within the Integrated Care Clinic, assist in coordination of medical services, health navigation, and provide health education to all members of the patients integrated care team, PCP, family and other natural supports. You will work in collaboration with the Department Leader to assure a high standard of care in integrated medical, health and wellness services. You will provide training, education, and support on medical, health and wellness concerns throughout the organization. You will work directly with patients with complex needs, clinical staff, onsite and offsite physicians, family, and other natural supports.
In addition, You will...
Oversee and implement medical care and preventative care services for patients with behavioral health and acute and chronic health conditions or at risk for such conditions. Provide direct care, education, and support to patients with complex physical and behavioral health needs.
Supervise medical assistants, Medical Care Coordinator, and other staff assigned within the medical clinic.
Hire and evaluate medical staff.
Oversee Integrated Medical Services (IMS) participation in huddles, as needed.
Identify and utilize evidence-based practices whenever available and appropriate.
Provide training and collaborative support to assure accurate and comprehensive health assessments as part of integrated care planning.
Requirements:
Active Registered Nurse licensure in the state of Pennsylvania.
Associate's degree in nursing with at least three (3) years of experience in a behavioral health setting required
Bachelor's degree in nursing with at least two (2) years of experience in a behavioral health setting required.
Valid PA Driver's License and ability to drive, travel, and provide community-oriented services.
Basic Life Support certified.
Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.
Preferred candidates:
Bilingual: English/Spanish preferred.
Have Knowledge of or experience with trauma-informed approaches preferred.
