This position will serve as an ambassador of care and a resource for patients who are seeking to stop using tobacco. The expectation from this position is that it will guide, educate, and coach a diverse population of participants in quitting tobacco by following motivational interviewing techniques.Minimum Required: High School Diploma or GED. Preferred: Associates Degree. | Minimum Required: 2 Years equivalent experience. Preferred: 3 - 5 Years equivalent experience.

Job Duties:
Screen and enroll patients in the tobacco cessation and reduction program
Assess patient outcomes and document participant data
Provide continued patient support to help prevent relapse
Offer brief counseling and motivational interviewing to help patients decide to quit, obtain cessation medication, and sustain abstinence
Provide subjective advice
Connect patients to resources such as the 1-800-QUIT-NOW hotline
Monitor nicotine replacement treatments and patient journey progress
Schedule future check-ins with patient via phone, email, or MyChart
Optimize and standardize the smoking cessation program
Skills:
Computer skills and dexterity for data entry, analysis, and retrieval of patient information

Effective verbal and written communication skills
Building rapport and patient engagement
