JOB SUMMARY  

The Clinical Nursing Manager, reports to the Program Director, IDD and is a member of the DC I/DD Leadership Team.  Further, is responsible for ensuring that the quality of the medical treatment provided meets the standards of care.  In addition will provide leadership, program support, direction, and supervision to all nursing staff, directly supervise the RN and/or LPN staff.

Budgeted: $94,250

RESPONSIBILITIES 


  • Work in coordination with the collaborating medical staff, Qualified Intellectual Disabilities Professionals (QIDP) and Residential Coordinator (RC) on providing additional medical oversight and review; 


  • Ensure competency among all nursing staff through the development of training schedules and materials with a focus on nursing best practice models as well as promoting health and wellness standards; 


  • Provide direct support to people with disabilities for routine health care, conduct various assessments, address minor illnesses/injuries and ensure proper follow-up; 


  • Make referrals to other medical professionals/facilities as appropriate; 


  • Serve as a member of the Interdisciplinary Team and participate in the Individual Treatment Plan (ITP) planning and implementation processes as required; 


  • Work with the QIDP’s and RC’s to ensure that adequate nursing personnel is scheduled at all times for all environments;  


  • Be available, on-call as scheduled, for nursing consultation and/or treatment of minor illnesses or injuries and render nursing assistance in medical emergencies. 


  • Develop rapport with each person receiving nursing care. Demonstrate respectful interactions at all times. 


  • Participate and/or render guidance in the initial admission, evaluation and treatment plan designed for each person admitted to this program. 


  • Support Direct Support Professional (DSP) staff in the provision of medical support for the individual. 


  • Participate as a member of the Interdisciplinary Team providing general health counseling, review of medical procedures, medication and medical options with the person, their family, guardian and staff. 


  • Ensure people and their families have coordinated medical appointments. 


  • Visit homes regularly as deemed necessary per the ITP to monitor the general medical status of people supported. 


  • Communicate with the other nursing staff regarding each person’s medical needs including routine, and well as, emergency treatments. 


  • Complete all necessary requirements related to MCIS-generated issues within the designated timeframes. 


  • Actively participate in the leadership of the monthly program management team meeting;  


  • Actively participate as a member of the I/DD Leadership Team. 


  • Performs other duties as assigned. 

Medical Services  


  • Work in coordination with the individual primary care physician and/or specialists to ensure that all individuals are receiving quality medical services. 


  • Ensure all medical follow-up for services and make all recommended changes. 


  • Work in coordination with all medical consultants to ensure that each individual receives quality medical services.   


  • Work in coordination with the pharmacy services to ensure that all individuals receive prescribed medication and supplements. 


  • Work in coordination with staff to develop a system to ensure all individuals attend all medical appointments. 


  • Participate in all required training related to the Healthcare Passport for individuals. 


  • Help coordinate all nursing-related information during program discharge or transfers.  


  • The Manager, Nursing Services will also ensure that all discharge teaching and staff training are completed and that the Health Passport is updated. 


  • Support implementation of all hospital discharge instructions. 

Medical Records 


  • Conduct periodic Quality Assurance (QA) reviews on all medical charts to ensure compliance and follows up with any needed corrections. Audit all nursing medical books routinely as designated. 


  • Actively participate in activities related to medication administration 


    • Conduct periodic medication administration reviews and medication passes to ensure compliance. 

    • Identify any errors and reports accordingly. 

    • Train, track LPNs in Medication Administration training and all required documentation including the proper completion of supervisory medication passes. 

    • Regularly monitor the Medication Administration Record (MAR) 

  • Maintain all appropriate medical documentation such as medical consultations, medical history, etc. 


  • Prepare medical summaries and attend ITP meetings as scheduled. 


  • Complete and/or delegate completion of identified assessments as needed at least annually for each person supported. 


  • Ensure all Healthcare Passports are complete and up to date.  


  • Develop and maintain Health Care Management Plans for all individuals in the program and ensure that appropriate staff are involved in the process.  Ensure up-to-date documentation of all follow-up consults, such as, Nursing Assessment, Glasgow Scale, AIMS, Fall Risk Assessment, Braden Scale, Vaccinations, Annual Health Form, Self-Administration of Medication Assessment.  


  • Monitor all adaptive equipment issues, ensure they are identified and addressed in a timely fashion.  


  • Ensure entries on the medical charts for all individuals are accurate and completed in a timely fashion. Make appropriate entries in nursing notes related to the care of the people supported. 

Training 


  • Provide required in-service training to staff in basic health care and hygiene skills. 


  • Train and monitor Trained Medication Employee (TME) trainings and document appropriately. 

Nursing Supervision/Leadership 


  • Hire qualified nursing staff for the program and maintains adequate staffing ratio for nursing. 


  • Develop nursing schedules for each home to ensure appropriate coverage based on the needs of the people living in the home. 


  • Conduct regular performance reviews for all RN and LPN staff as required.  


  • Develop training schedule for all nursing staff that focuses on nursing best practice and health and wellness standards. 


  • Ensure all nursing staff maintain all of their training and licensing requirements. 

EFFECT ON END RESULT 


  • To ensure needs of the person supported, as identified by the IDT, are met and individual satisfaction is achieved. 


  • To ensure that the program is operated in compliance within the Scope of Work, Volunteers of America Chesapeake’s policies and procedures, licensing and Medicaid Waiver regulations.

REQUIREMENTS 


  • A graduate of an accredited school of nursing with registration and license (RN) to practice nursing in the District of Columbia. 


  • Minimum of three (3) years of clinical experience working with persons with complex medical and health needs. 


  • Working knowledgeable of the rules and regulations of the relevant service environments including Waiver, Residential Habilitation, and Individualized Day Support services; and adhere to the program guiding principles. 


  • Experience working with people with disabilities is required.  


  • Ability to document all healthcare and medical needs as required by all licensing and monitoring entities and contribute meaningfully during internal audits and external monitoring visits including Evans Class Monitors, Quality Trust, Health Regulation and Licensing Administration (HRLA), Department of Health (DOH), Provider Certification Review (PCR), Department of Disability Services (DDS), Developmental Disabilities Administration (DDA) in the District of Columbia. 


  • Experience supervising nurses (i.e. RN’s and LPN’s) within the Waiver model of services; 


  • Proven experience in a residential setting serving people with intellectual/developmental disabilities or with other types of special needs populations (rehabilitation settings, nursing homes, psychiatric hospitals, other behavioral health settings, etc.), working with people who are medically fragile and/or have physical limitations; and/or 


  • Proven experience in working with Evans Class Monitors, Quality Trust, HRLA, DOH, PCR, DDS, DDA in the District of Columbia. 


  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  


  • Ability to successfully navigate an electronic health record system 


  • Ability to write routine reports and correspondence.  


  • Ability to perform basic computer and office equipment skills. 


  • Ability to effectively present information in one-to-one and small group situations to people supported, families, guardians, and other medical professionals. 


  • Has working knowledge of community resources and a plan for accessing those resources.

PHYSICAL REQUIREMENTS

  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment. 


  • The physical ability to move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment. 


  • The physical ability to lift, transfer, carry, push, pull a person served when required through routine assistance or during an emergency.
