JOB SUMMARY
The Case Manager will provide key support for each participant through the collaborative development of individual education plans (IEP)s, monitoring progress, and coordinating critical support services to ensure participants experience successful outcomes.
RESPONSIBILITIES
The Case Manager will recruit one candidate as a youth mentor throughout their enrollment in Youth Build.
At program launch, the Case Manager will immediately develop a template for participants IEP document, review enrollment rosters, and build connections with community providers for support services.
The Case Manager will establish rapport with individual youth, create IEPs with youth,
coordinate support services and attend field trips with participants.
The Case Manager will conduct weekly mentoring sessions with each youth and address any roadblocks participants encounter during the program as youth are placed in advance training or employment.
The Case Manager will provide regularly scheduled follow-up visits to ensure smooth transitions for youth.
Complete other duties as needed and requested.
REQUIREMENTS
Bachelor’s degree in social work, sociology, psychology, counseling or related field.
One year of experience working with young people, youth in foster care system as a social service provider preferred or relevant experience may be substituted for degree.
Experience working with the African American and Latino community; knowledge of Newport News community resources and social service providers is preferred.
This position requires driving your personal vehicle for company business. Must be privileged to drive according to the criteria set forth by Agency MVR guidelines.
Valid driver’s license in jurisdiction of residence
Vehicle liability coverage if driving personal vehicle (on occasion, may be required to transport clients in personal vehicle)
May be required to drive a van
Acceptable Criminal background check, OIG Exclusion check
Negative Drug Screening
CPR/First Aid Certification must be acquired and maintained once employed

The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.

Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
Operating office equipment requiring continuous or repetitive hand/arm movements.
The ability to remain in a sitting position for extended periods of time.
