The Role: Family or Internal Medicine Physician / Diabetes Clinician
Health by Design, a Medici company is seeking a full-time Family or Internal Medicine Physician for a newly created Diabetes Clinician role. Will provide focused diabetes care to a unique, all adult patient population. This individual will travel between four clinic locations (will not typically work at more than one location per day) caring for retired San Antonio Police and Fire Fighters, as well as other First Responders. Our beautiful state-of-the-art health and wellness centers offer a variety of services that patients can take advantage of to help them in their journey to better health including on-site health and nutrition coaching, cooking classes, exercise classes, mental health care, physical therapy and more! Our physicians practice in a value-based care model (NO fee-for-service) with LOW patient volume and no productivity metrics. This role will be well-supported with experienced Registered Nurses and Licensed Vocational Nurses, as well as working collaboratively with other providers. Monday Friday, 8:00am 5:00pm. No late hours, no call and no after-hours charting! Clinics are located in the Westover Hills, Fair Oaks, Roosevelt and Schertz areas.
What Youll Do
Provide medical patient care by interviewing, examining, and treating clinical patients in order to meet their needsAdvise, order, and perform tests and interpret the resultsDiscuss diagnoses with patients, explain treatment options, and educate them on medications, lifestyle and dietary changes and other preventative measures.Carries out follow-up appointments, monitor patient progress, adjusts treatment plans and medications, and updates patient records.Educate patients on wellness, prevention, and early detection by providing materials and resources to the patientsDetermine which referrals are required based on examination and patient needsDetermine level of urgency of follow-up, referral/consultation appointmentsPrescribe medical treatment and clinical drugs to patientsOrder studies, tests, and ancillary servicesDocument all services in patient medical recordReview on a regular basis long term cases that require ongoing medical attentionConsult with Physician Lead, Chief Medical Officer and other professionals or staff as needed regarding patient care, assessment, and educational issuesRemains up to date on current practice developments, trends, research, and technology.Assists as needed in the resolution of complaints, requests, and inquiries from patientsAssist in the development, production, and presentation of medical education, material, lectures, and mixed media presentations for patient and medical staffMaintain confidentiality of all patient information according to federal guidelines and regulationsMay be assigned to multiple clinic sites as mutually agreed upon with medical leadership.May require on-call responsibilities
