What You Will Do:
Develop, organize, and direct a hospital-wide care management program focused on utilization, discharge planning, and coordinated care.
Supervise the activities of hospital staff in inpatient care coordination, social work, utilization review, and discharge planning.
Collaborate with the Medical Director of Care Management and the Director of Documentation Quality and Care Transitions on initiatives for denial prevention, readmission reduction, and palliative care.
Guide hospital staff, medical personnel, patients, and families through education, consultation, and ongoing support.
Minimum Qualifications:
Master's degree or higher in a healthcare-related field (nursing, social work, healthcare administration, or medicine).
Maryland-recognized licensure in clinical specialty if applicable.
Minimum 5 years in care management or related field, with 2+ years in a leadership role in case management, utilization review, or similar areas.
Extensive understanding of payer mechanisms and clinical utilization management.
Strong interpersonal skills with a track record of effective interdisciplinary collaboration.
Knowledge of relevant regulatory standards and proven experience interacting with external review organizations.
Pay Rate: $54.94-$82.40
Pay is based on experience, skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
