About the job:
We are looking for an experienced Social Worker with a passion for geriatrics. Our Social Workers are key players in providing support and service to our residents and their families. Our careers offer opportunities to make a positive impact in the lives of seniors every day.
Essentials:
LTC case management
Discharge planning
MDS assessments
Care planning
Providing resources, and referrals to residents/families/staff members
Coordinates resident admissions
Coordinates and leads transfer and discharge team
Completes assessments of residents as necessary for Care Planning, MDS, RAPS, PPS and other required documents
Develops and promotes the department through marketing efforts throughout the professional and social community
Required Qualifications:
Bachelor's degree in Social Work or related degree in social sciences.
Twenty-one (21) years of age, meet the MVR organization standards, and have a valid state driver's license
Ability to read, speak and write fluently in English
Preferred Qualifications:
1 year of experience with LTC
