The Department of Medicine - Pulmonary has an exciting opportunity for a Pharmacist. Incumbent will remain committed to implementing an outpatient clinical pharmacy services program
Collects data on the impact of the clinical pharmacy services provided
Will be responsible for learning CFF Port Patient registry concepts, methods, data entry, and reporting on an ongoing basis Tracking and reporting on the patient assessment, subsequent assessments and follow-up, provision of interventions, and their outcomes
Documenting in the annual progress reports, the CFF Port Registry, as well as in the patient(s) Electronic Medical Record (EMR) Updating and notification of any changes to the CF Patient Registry protocol to adhere to CFF guidelines and protocols
Learn and advise about the effectiveness and safety of drug combination in patients
Assist with Prior Authorizations of medications and needed follow up Coordinate pharmaceutical care with third party vendors and/or specialty pharmacies
Assist in establishing Standard Operating Procedures (SOPs) and other meaningful guidelines for our center from a pharmaceutical perspective.
Minimum Requirements:
Pharm.D. degree
Minimum 1 year of relevant experience
Skill in collecting, organizing and analyzing data.
Ability to recognize, analyze, and solve a variety of problems.
Ability to exercise sound judgment in making critical decisions.
