Job Description:

We are looking to expand our MDS Department by adding an additional MDS Coordinator. This registered Nurse position is responsible for coordinating MDS plans within the beautiful Eastern Nebraska Veterans Home.


Schedule: Monday-Friday 0800-1630


Wage Information: $37.825-$49.474/Hour based on nursing experience.(Includes $3.00/hour building differential)



JOB DUTIES:



  • Assist with the development of an action plan for MDS Coordination

  • Serve as "resource person" when questions arise from those staff completing the MDS assessment, raps, and triggers

  • Create and maintain a schedule for annual and quarterly reviews

  • Assign MDS assessment to staff when a Member experiences a "change of condition"

  • Assist with the developing and presenting of in-services to all departments on the MDS process

  • Assist in defining responsibilities of various departments in completing the MDS assessment form

  • Work with CCP committee to incorporate the MDS assessment into the comprehensive plan of care

  • Develop and implement monitors to assure compliance with the MDS completion actions plan; monitors will then be sent to the DON for review and any necessary follow up




Requirements / Qualifications:




Minimum Qualifications: A current license to practice as a registered nurse in Nebraska or authority based on the Nurse Licensure Compact to practice as a registered nurse in Nebraska.



Preferred Qualifications: Prior experience writing MDS and Care Plans



Other: Must pass background checks and complete pre-employment drug screening.



KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of nursing theory and practice; accepted standards of care; policies and procedures governing nursing care; sanitation and infection control standards, policies and procedures and techniques of providing quality nursing care. Ability to lift up to 50 lbs. occasionally and push/pull occasionally up to 50 lbs. on a daily basis; to frequently bend at the waist and neck and frequently twist at the waist and walk or stand up to 50% of the shift.
