Job Summary: We are seeking a compassionate and organized Case Manager to join our team. The successful candidate will be responsible for coordinating care and support for residents, ensuring they receive personalized and effective services. This role requires excellent communication skills, the ability to work collaboratively in a dynamic environment, and a commitment to enhancing the well-being of our residents.
Key Responsibilities:
Resident Communication: Engage regularly with residents and their families to discuss Plans of Care, treatment protocols (PT, OT), and any concerns, ensuring they feel safe and secure in our care.
Team Collaboration: Actively participate in morning meetings and stand-ups to facilitate effective communication with the care team.
Monitoring and Assessment: Continuously assess and adjust resident care plans based on changing needs and conditions, ensuring optimal support.
Work Organization: Prioritize daily tasks by evaluating the needs of new, current, and discharging residents within your area of responsibility.
Documentation: Complete necessary documentation accurately and timely, including utilization reviews, to ensure appropriate, cost-effective care.
Care Coordination: Liaise with residents, healthcare providers, financial services, and third-party payers to coordinate seamless care.
Admissions and Discharges: Oversee all admission and discharge activities, ensuring a smooth transition for residents.
Referral Management: Coordinate referrals to and from our facility to enhance service delivery.
Relationship Building: Foster positive and effective relationships with residents, families, staff, and professional colleagues.
Qualifications:
Education/Experience: Bachelor's Degree in Nursing or Social Work. RN license or LVN/LPN license preferred, but not required.. Minimum two years of clinical nursing experience preferred.
Knowledge: Familiarity with Medicare, Medi-Cal, and Medicaid programs and benefits.
Licenses/Certifications: Valid LVN/LPN or RN license in the state of employment is preferred; CCM certification is a plus. Must maintain all required continuing education/licensing and remain in good standing with the Department of Public Health.

Time Management: Strong organizational skills to manage workload efficiently in a high-volume environment with shifting priorities.
Physical Demands:
Ability to stand/walk frequently, sit occasionally, and lift up to 50 pounds.
Frequent reaching, pushing/pulling, and effective communication skills (talking, hearing).
Occasional climbing, balancing, kneeling, and visual acuity (close and distance perception).
Work Environment:
