Works cooperatively with the management team to schedule and complete medical appointments for individuals served in a timely manner.
Completes all necessary medical follow ups and required post medical appointments in a timely manner.
Works with medical providers and pharmacy to ensure all medications are ordered and filled as prescribed.
Completes weekly medication reviews accurately.
Review residents' medical files weekly.
Assists with emergency situations, communicates and documents such incidents in accordance with Agency policies and procedures.
Uses technology for the completion of specified job duties.
Attends work regularly according to assigned work schedule and in accordance with Agency policy.
SECONDARY FUNCTION(S):
Works cooperatively and effectively with stakeholders while meeting the needs of individuals served and the programs.
Provides personal care to persons served during specified time periods.
Available to assist Residential Managers for needed support.
Provides transportation to the individuals served when needed.
Works alongside Residential Manager to ensure the Emergency Food and Supplies Kit, OSHA kit, and First Aid kit is sufficiently stocked.
Attends and participates in in-service training, staff meetings and other activities to facilitate professional development.
Works cooperatively with others including all staff, supervisors, administrators, co-workers, individuals served, community professionals, customers, vendors and the public.
Assumes other duties, responsibilities and special projects as needed.
EDUCATION: Associates of Arts or Science degree from an accredited college or university with emphasis in Human Services. Bachelor's degree preferred. EXPERIENCE: Two years' experience working with individuals with intellectual disabilities. NOTE: At the discretion of the Personnel Officer, additional related experience and/or education may be substituted in lieu of the requirements specified above under Education and Experience.What's in it for you?
