Provide Diabetes Education and Care to Patients and Staff in a variety of settings- Including ED, Inpatient, Critical Care, and Same Day Surgery.
Collaborate with engaged providers and hospitalist team to deliver quality care for optimal outcomes.

1. Assesses, plans, implements and evaluates learning needs of the inpatient and outpatient population a. Provides inpatient programs for nursing personnel b. Evaluates and revises as needed all resource materials for diabetes education for nursing staff. c. Utilizes research as a basis for providing and improving diabetes education. d. Participates in Care Team Rounds e. Consults with nursing staff as requested for inpatient referrals. f. Provide Diabetes Education to inpatients per assessed needs g. Provide individual and group outpatient Diabetes Education
2. Participates in community diabetes education . Plans, implements and evaluates outpatient education programs a. Participates in health screenings and/or local support group meetings. b. Participates in Diabetes Advisory Board meetings c. Provides diabetes education to women with gestational diabetes d. Assists and educates patients with the initiation and ongoing maintenance of insulin pumps e. Participates in planning, implementation and evaluation of camps/events for children with diabetes. f. Under the direction of the physician, adjusts insulin doses for optimal glycemic control
3. Works collaboratively to market a comprehensive community diabetes program . Represents WDH at community events as requested a. Provides education to primary care practices b. Provides diabetes education to off-sites as contracted. c. Participates in assuring adherence to ADA recognition standards and application process completion
4. Able to manage technical equipment associated with the Diabetes Education program. . Demonstrates skill in performing quality control tests and patients tests using a whole blood analyzer for blood sugars a. Demonstrates skill in using a variety of current blood glucose monitoring equipment and available software for downloading of results. b. Demonstrates skill in using audiovisual equipment necessary to present group education. c. Demonstrates skill in using equipment for continuous blood glucose monitoring d. Demonstrates skill in using equipment for HgbA1c monitoring
Qualifications
Experience Minimum Required
• Over three years experience in an acute care hospital; direct patient diabetes education
Experience Preferred/Desired
• Over one year experience as a Diabetes Educator; proven grant writing experience; curriculum development experience.
Education Minimum Required
• Associates Degree in Nursing
Education Preferred/Desired
• Bachelor's or Masters Degree in Nursing
Special Skills Minimum Required
• Able to present in group settings to general public and/or to other health care providers.
Licensure and/or Certifications Required
• Diabetes Education Certification within 3 years from date in job • Registered Nurse License
