The role of the Clinical Care Coordinator serves as an integral part of the Structural Heart Team by coordinating care for patients undergoing transcatheter heart valve procedures. Working closely with Interventional Cardiologists, Cardiovascular Surgeons, and Nurse Practitioners, the Structural Heart Nurse Coordinator is expected to navigate care for patients potentially undergoing complex structural heart interventions. Working collaboratively and cooperatively with other healthcare professionals, the Structural Heart Nurse Coordinator is a resource for ensuring continuity of care, overall quality of care, and patient satisfaction.

Position Summary:

Performs safe, effective clinical care and coordination of care for children / adolescent, adult, and older adult patients utilizing the nursing process. Demonstrates advanced nursing skills aimed at ensuring optimal patient care, effective communication and unit efficiency. Supports nursing research by participating in research activities, when appropriate.

Position Key Accountabilities:

1. Effectively manages and coordinates all aspects of the patient care process, and documents patient care and outcomes per policy.

2. Participates in regularly scheduled team conferences, as required, and collaborates with related health care providers in treatment, discharge planning and the provision of care.

3. Implements and monitors patient care plans, navigates patient through the process and maintains and updates patient records as necessary. Ensures program and patient compliance.

4. Functions as the nursing liaison with physicians, other multidisciplinary team members, and community agencies/resources.

5. Serves as a resource for staff, to include the provision of information concerning evidenced based practice for procedures and other patient care developments.

6. Participates in the development of nursing policies and instructional materials for staff/students.

7. Prepares and maintains reports concerning operations, as requested.

8. Assists in resolving interdepartmental / intradepartmental clinical problems and/or patient concerns, and leads identified performance improvement initiatives for the unit (chart reviews, PI initiatives, etc).

9. Provides coaching and timely performance feedback to staff, as needed, and reports to supervisor for continued follow-up with staff.

10. Assists supervisor with coordinating patient care conferences, etc., as indicated.

11. Ensures timely completion of monitors / logs for quality control and performance improvement monitoring on the unit.

12. May manage Human Resources activities of department in regards to: recruiting and selection, hiring and termination, training, professional development, mentoring, counseling, performance evaluations and salary planning.

13. Manages other duties as assigned.

Certification/Skills:

Current nursing licensure in the State of Texas.

Minimum Education:

Bachelor's degree in nursing plus three years nursing experience, OR
Associates degree in nursing plus five years nursing experience, OR
Nursing diploma (3 year program) plus four years nursing experience.

Minimum Experience:

Three (3) years of nursing experience with BSN
Four (4) years of nursing with a Nursing Diploma
Five (5) years of nursing experience with ADN

Physical Requirements:

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or a negligible amount constantly to move objects.

Residency Requirement:

Employees must permanently reside and work in the State of Texas.
