Job Overview
The Social Work Manager position is responsible for the general oversight and management of PACE Social Services, including initiation, planning, implementation, and improving social services, policies, procedures, to provide quality patient care at all AltaMed PACE Centers. Ensures the efficient and effective operations of social services while requiring compliance with all state, federal, and local regulatory laws, standards, and protocols. Serves as a key resource and subject matter expert for social work staff and ensures efficient and effective operations workflows, and processes while requiring compliance with all state, federal, and local regulatory laws, standards, and protocols. Provide leadership for social work function while working collaboratively with the PACE operations and medical leadership, to ensure the achievement of PACE strategic goals. Supports the administrative leadership team, in interviewing, hiring and onboarding and training of Social Work Supervisors, Social workers and Senior Care Advocates to ensure social work staff is knowledgeable, skilled, and competent to provide quality care in the PACE model. Providing the overall leadership to train, coach, and develop Social Work Supervisors, Social workers and Senior Care Advocates, and managing operations and services that add value and are aligned with organizational performance and quality measures. The Social Work Manager represent social work practice within the interdisciplinary team approach to ensure implementation of quality, cost-effective, standardized, and efficient processes, which support coordination of participant-centered care goals at all PACE sites.
Minimum Requirements
12. Master’s degree in Social Work (MSW) from an accredited university required.
13. Minimum of 5 years of experience in the clinical setting required.
14. Minimum of 2 years of supervisory experience required.
15. Current BLS for Healthcare Professionals certification required.
16. Geriatric experience as required by CMS PACE regulations.
17. Experience in working with Medi-Cal, Medicare and PACE regulations.
Compensation
$102,954.74 - $128,693.43 annuallyCompensation Disclaimer

The range is subject to change.
