The Hospice RN Case Manager provides skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice. The Hospice RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling in accordance with the interdisciplinary team care plan.
Qualifications
Graduate of an approved, accredited school of professional nursing required; BSN preferred
Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts required
Current CPR certification required
Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred
Demonstrated excellent verbal and written communication skills required
Demonstrated strong interpersonal, organizational, and time management skills required
Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required
Current driver's license, a good driving record, and reliable transportation required.
