Job Description:

JOB SUMMARY:
Organizes and evaluates patient medical records. Reviews medical records for accuracy and completeness. Responsible for filing and retrieving medical records.

JOB SPECIFIC DUTIES AND RESPONSIBILITIES:

Pulls and files paper medical records as needed.
Reviews documents received to determine if they require scanning.
Preps documents needed to be scanned according to policy and procedure.
Indexes medical record documents and ensures accuracy of scanning level (patient, encounter, order). This includes reviewing document type master index to identify the type of document and determining whether or not document is appropriate to be incorporated in the EMR.
Suspends documents without identifiable document type or patient issues for quality review.
Reviews documents to insure appropriate frame, resolution, orientation, etc. and adjusts accordingly.
Responds to phone calls from medical and support personnel requesting medical records.
Logs medical record requests into database.
Determines records to be released by reviewing requestor information in accordance with HIPAA guidelines and obtaining pertinent patient data from various sources, including electronic, off-site, or physical records that match patient request.
Responds and processes requests from physician offices on a priority basis and fax information to the physician office.
Other duties as assigned
Maintains strictest confidentiality.


OTHER JOB REQUIREMENTS: May be required to travel to satellite clinics.

EDUCATION, CERTIFICATION, LICENSURE and REGISTRATION: High school diploma or equivalent

EXPERIENCE, KNOWLEDGE, SKILLS and ABILITIES:

Minimum two years' medical clinic experience preferred
Knowledge of medical records work procedures preferred
Knowledge of computer applications
Courteous and professional telephone skills
Ability to multi-task


REASONING ABILITY:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in written, oral, or schedule form.


INTERPERSONAL SKILLS:

Must interact and communicate both verbally and in written form.
Must interact and exchange information regarding patients with physicians and other departmental personnel, and outside agencies on a frequent basis while respecting the confidentiality of patient information.
