In our statewide Corrections and Community Reentry Services, we provide specialized case management services to help people prepare to return to their families and communities after they've been incarcerated. We work both inside prisons and in communities to conduct screening and assessments for substance use disorders, place clients into treatment programs, and provide support with such activities as insurance enrollment, obtaining a state ID, and finding employment resources.

We are currently looking for full-time – Case Manager

Starting at $41,600 – contingent upon experience, education, etc.


Summary: This position will provide addiction intervention, AODA counseling and recovery support services to clients and their families. This position is responsible for bringing services, agencies, resources and other people together to assist a client in achieving established goals.

  • Provide direct services and coordinate other services, resources and people to assist clients in achieving their established service plan goals
  • Screen and assess clients for their appropriateness for agency services
  • Adheres to the service hour protocol established by the agency
  • Support engagement in drug treatment, community, etc.
  • Counsel and aid family members to assist them in understanding, dealing with, and supporting the client or patient
  • Facilitate individual and/or group orientation or educational sessions (as required)
  • Represent clients and agency at staffing and court hearings, probation, parole and provider networks
  • Provide referrals for identified needs of the client and crisis intervention when needed
  • Document the results of assessments, service plans, reports, progress notes, discharge summaries and other client-related activities including accurate reporting of services
Requirements:

Our Ideal candidate:

  • Bachelor's degree preferred in human services, social work, psychology or other related fields
  • One or more years' experience in human service activities; or any equivalent combination of training and experience
  • Knowledge of human behavior for the assessment and signs and symptoms of substance abuse disorders. Specific knowledge necessary for working with special populations
  • Knowledge of the Code of Ethics for substance abuse professionals
  • Knowledge of the federal drug and alcohol confidentiality law (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act
  • Knowledge of the criminal justice system
  • Must be available to attend meetings and meet the needs of the program and/or clients throughout the city, counties or wherever needed (a valid driver's license, current auto insurance and reliable automotive transportation are required)

The agency currently offers hybrid work schedules that combines in-office and remote work. Employees are required to report to the work site three days per week with the option to work up to two days remotely.
