Great opportunity for a Therapeutic Mentor to join our dedicated Quincy In-Home Therapy team of professionals! Office is based out of Braintree.
A Therapeutic Mentoring provider delivers services in accordance with an existing outpatient, ICC, or in-home therapy or treatment plan that is developed by the outpatient, in-home therapy provider or intensive care coordinator with input from the parent/guardian/caregiver, and youth whenever possible. This includes supporting, coaching, and training the youth in age appropriate behaviors; interpersonal communication; problem-solving and conflict resolution; relating appropriately to other children and adolescents, as well as adults, in recreational and social activities. Completes all clinical documentation and service plans on-line using AVATAR Electronic Health Record System.
Provides active efforts to engage clients who have traditionally been difficult to engage in service.
Collaborates with other members of the treatment team.
Responds to referrals in a timely manner.
Performs service plans.
Documents all therapeutic services on the daily progress note.

Required Skills
A bachelor's degree in a human services field and one year experience working with children/adolescents/transition-age youth or
An associate's degree in a human services field and one year experience working with children/adolescents/transition-age youth or
A high-school diploma or GED and a minimum of two years' experience working with children/adolescents/transition-age youth
Valid MA State driver's license, safe driving record and access to personal vehicle for client transportation
CPR Certification - can be completed after date of hire
Required Experience
Minimum of one to two years' experience working with children/adolescents/transition-age youth depending upon education
Must be a minimum of 21 years old
Good communication skills (both written and oral)
Must demonstrate the following characteristics: flexible, team player, non-judgmental, ability to grow and learn in the position, ability to provide support and quality care for the well-being of clients and families served.
Experience utilizing Electronic Health Record preferred
Computer proficiency and ability to learn new systems and applications
