This position is with the UNC Addiction Medicine Program, which provides Addiction and Psychiatric services to those with substance use disorders. The Addiction Medicine Program is comprised of the UNC Substance Treatment and Recovery (STAR) clinic Hub, co-located with the UNC Alcohol and Substance Abuse Program (ASAP). The STAR clinic is the HUB of a HUB and SPOKE model and functions to stabilize patients with substance use and then transition the patients for ongoing, maintenance treatment at a Community Clinic (the SPOKE). STAR clinic Hub is also the training site for the UNC Addiction Medicine fellowship, which trains up to 8 physician fellows annually, and provides training to medical students and residents. The Addiction Medicine Program provides psychiatric and psychosocial assessment, medication management, psychotherapy and family services to serve individuals with substance use disorders. Additionally, the Addiction Medicine Program is involved with expanding addiction treatment/services across the University and in the Community. This position will focus on transitions, including: identifying patients in the ED/hospital or community with substance use disorders, linking these patients to treatment, assisting with their transition to STAR clinic, providing case management/care coordination, and assisting the patients with transition to the Community clinic
In connection with the UNC Addiction Medicine Program, the Social Work Practitioner will assist with patient transitions both in the inpatient and outpatient settings, to include engagement with hospitals, Emergency Departments, Substance Use treatment programs and community providers. The Social Work Practitioner will assess case management needs and assist, as needed. This position will be responsible for assessment, planning and coordination of treatment services provided by the UNC Addiction Medicine Program, in addition to updating the treatment plan to meet a patient's changing needs.
Responsibilities are listed below and include services provided both for the inpatient and outpatient settings:
Bachelor's degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.
Previous experience in care management with identified patient population (Substance Use Disorders and Mental Health) required.
A valid driver's license, and access to reliable transportation.The ability to drive 29ft mobile medical vehicle (CDL not required).Licensed to practice in the state of North Carolina is preferred.
