About the job:
The Community Outreach Coordinator will focus on promoting and growing our PACE ministry through community contact and integrating Senior Community Care as a vital member of the healthcare and long-term care network

Business-to-consumer (B2C) marketing, outside sales or community outreach experience
2+ years of experience conducting presentations to community stakeholders and leaders
Proficiency MS Office (Word, Excel, Power Point, Outlook, Teams)
Ability to work core business hours, Monday - Friday, 40 hours and overtime, weeknights, and weekends, when required
Active driver's license with access to reliable transportation that will enable travel to the community and/or sites within a designated area
Travel up to 75% of the time within this market area
Live in/within commutable distance to the Senior Community Care Adult Day Health Center
Preferred Qualifications:
Bachelor's or Master's degree
Experience building and maintaining provider and community relationships
Experience engaging with health centers and/or provider offices
Experience working with managed care organization (MCO)
Experience engaging with medical providers and medical staff in Medicaid outreach
Experience working with communities of all different ethnicities, cultural backgrounds, diverse populations and/or underserved communities
Established professional relationships with non-profits, community sources CBO's, religious/faith-based organizations FBO's in designated sales territory
Essentials:
For the assigned PACE Organization site, acts as Senior Community Care's Outreach representative at community events, health fairs, community meetings and activities of the PACE Organization
Establishes our presence in the community by outreaching virtually, telephonically and in-person by creating a full calendar of outreach opportunities, securing participation in established events, and setting up and conducting our own activities
Develops and strengthens relationships with key partners, including community-based partners, faith-based organizations, provider groups, governmental agencies and more, who also serve our existing and potential participants and other Medicaid eligible individuals
Sets up event participation, including setting up booth/table, collateral materials, engagement interactions, and other supplies for exhibits and meetings with community
Supports health education activities programs and campaigns in clinics, community centers, farmer's markets, and partner locations
Assists in tracking, organizing, and ordering collateral and health education materials and giveaways
Collects and manages outreach and event data to track effectiveness of activities and other key metrics
