Job Description:

Job Summary
Provides dental education and preventative dental services, assist in the coordination of dental care, navigation of the healthcare system, and patient tracking and follow-up including helping obtain healthcare appointments and transportation, as well as, other support when necessary. Map out the social and health support networks within a community to insure that parents and guardians of high-risk children know how to access available resources. Utilize culturally relevant methods to target specific populations with the use of individual behavior change strategies that can be used to improve an individual's oral health status.


Job Specific Duties


  • Responsible for the coordination and delivery of educational activities involving providers in the NCHS including the Pediatric Care Center, School Based Health Clinics, Emergency Department, and Mobile Unit.

  • Works under the supervision of medical and dental providers and within the confines of state dental practice acts to provide limited clinical services, including screenings, fluoride treatments, placements of sealants, coronal polishing, and radiographs.

  • Collects information to assist healthcare providers in triaging patients.

  • Serves as the liaison for high-risk patients identified at NCHS facilities who require follow-up care with friendly interaction and communication.

  • Performs patient tracking and follow-up. Assist in helping obtain healthcare appointments and transportation, as well as, other support when necessary.

  • Provides dental health education and help people develop goals to enhance their oral health.

  • Helps patients navigate the complexities of the health care system.

  • Utilize culturally relevant methods to target specific populations with the use of individual behavior change strategies that can be used to improve an individual’s oral health status.

  • Supports the development of educational materials for target populations and healthcare providers.

  • Prepares reports for internal purposes, external agencies, and donors.

  • Collects data to support the preparation of grants and contracts.

  • Monitors CDHC program goals and monthly performance.

  • Maintains appropriate records and documentation for internal and external regulatory purposes.

  • Reviews, maintains, and documents patient information in the computer thoroughly, accurately, and in a timely manner.

 

Minimum Job Requirements

 


  • Graduate of an accredited school of Dental Assisting or Expanded Duties Certified

  • Community Dental Health Coordinator Certificate within 7 months in position

  • Expanded Duties Certification - maintain active and in good standing throughout employment

  • Nitrous Oxide Certification - maintain active and in good standing throughout employment

  • CPR - American Heart Association AED - maintain active and in good standing throughout employment

  • 3-5 years Experience in dental assisting
