Position Description

Employees in this classification are responsible for performing a wide variety of patient care activities in an assigned specialty or sub-specialty outpatient clinic. Work is performed under the direction of a licensed staff nurse and/or physician as part of a total health care team in providing complete evaluations to patients. Work is performed under usual clinic conditions, necessitating a high degree of contact with patient, families and clinic personnel.

Major/Essential Functions

  • Reports data, identifies and reports alterations in patient responses to assist in the identification of problems and formulation of goals/ outcomes and patient-centered plans of care in collaboration with patients, their families, and the interdisciplinary health care team.

  • Monitors nursing processes to determine the physical and mental health status, needs, and preferences of patients and their families, based upon interpretation of comprehensive health assessment findings.

  • Monitors communication of patient data using Electronic Health Record to support decision making for delivery of optimal patient care.

  • Works with faculty and resident physicians to submit medication refills, schedule procedures, obtain MRI or procedure and lab results then report findings to physician. Assist phyisicns to develop, implement, and evaluate teaching plans for patients and their families to address health promotion, maintenance, and restoration.

  • Inspects examination/treatment rooms to ensure they are properly stocked and compliant with infection prevention and control policies. Prepares and operates necessary equipment and instruments for examination, treatments and procedures. Sterilizes and disinfects medical equipment and instruments.

  • Complies with the identification of patient needs and assist to resolve patient needs or requests within 24-48 hours. Implements and facilitates continuity of care.

  • Evaluate and report patient outcomes and responses to therapeutic interventions. Complete proper follow up on patient care issues/messaging.

  • Patient Intake - Calls patients back from lobby to exam room. Takes vital signs and measurements then accurately documents in patient EMR. Creates patient in-take form. Reconciles patient medications and updates mediation list as needed.

  • Receives all patient telephonic communication. Monitors and responds to all communication between clinic staff, nurses and physicians. Priority is given to patient triage. Documents all communication regarding patient care. Ensures staff are properly trained and are implementing departmental protocols. Telephone communication includes the following: Providing answers to questions concerning health problems, disease and treatment in a caring and informative manner. 

  • Weekly reviews medical supplies to ensure properly stocked. Notifies supervisor when supplies are low. Ensures no perforated boxes are in clinic or nursing area to include exam rooms and supply closet.

  • Completes Annual Nursing Core Competency Training and Skills Fair. 

  • Performs investigation of accidents/ injuries and generates occurrence reports for unexpected outcomes. 

  • Monitors schedules, room utilization and clinic patient flow to ensure maximum efficiency. Calls patients back to exam room in timely manner. 

  • Ensures physician orders for tests, referrals, and/or lab tests are in the patients EMR. Contacts ordering physician to advise of critical results.

  • Will supervise CNA and CMA to ensure punctuality and attendance expectations.

Required Qualifications


Education: Completion of a Board of Nursing accredited program of vocational nursing or scheduled to complete a program of vocational nursing within 120 days of applying for a position.

License/Certification: Current Vocational Nursing license in the state of Texas or the attainment of a temporary permit from the Board of Nursing to practice vocational nursing pending full licensure in the state of Texas. New graduates must pass the National Council Licensure Examination for Practical Nurses (NCLEX- PN) on the first attempt.
