JOB RESPONSIBILITIES




  • Strategy Program/Project Development- Poses specialized experience and knowledge to collaborate with leadership, internal stakeholders, and community partners to provide strategic leadership in designing and implementing community and institutional initiatives. Demonstrates deep understanding of the impact of community-based influences, including social and structural determinants of health impact on child health outcomes. Serves as internal and community resource, liaison and expert to child health Equity initiatives. Collaborates with existing programs and projects to ensure optimal workflow and collaboration among all internal and external stakeholders. Supports strategic vision and alignment of innovations in models of care across clinical, community, and educational areas of need.



  • Project/Program Planning- Co-develop and plan internal/external processes within an equity framework to understand causes and needed clinical-community response (working closely with frontline teams, DEI, external experts, and community-based organizations). Work with Quality Excellence and sites of care to prototype and scale successful interventions to close those disparity gaps. Collaborate with community partners to create and implement initiatives that will promote systems changes in pursuit of improved well-being outcomes. Outreach and partnership with other Child Health Equity Centers and equity research experts (e.g., SIREN) to identify shared agenda and strategies.



  • Operational support and expertise- Communicates and promotes a shared purpose and narrative around a compelling and integrating vision (w/Internal and External leaders). Provides operational leadership in the process to co-develop measures and measurement systems across hospital and community (eg, 'Thriving'). Builds a strategy and supports execution of collaborative quality improvement & data capability across city partners (w/AC). Collaborates with Center leadership to design and implement process for operational planning, annual and quarterly reporting, and processes for cross-work integration for equity initiatives. Develops and implements policies and procedures for improved utilization and quality of programs, projects, and operations. Works with Center leadership and marketing and communications specialists on news articles, social media updates, and press releases. Provides support in the preparation and submission of external and internally funded projects via EPAS or online grant application portal as specified by the sponsor and maintain tracking. Collaborates with the necessary internal departments/divisions, partners, and external organizations and agencies needed to meet objectives.



  • Data Analysis and Quality Improvements- Collaborates with data and QI teams to create and execute project plans that will lead to effective gathering and interpreting of equity data, and improvement plans to improve outcomes. Coordinates the gathering and interpreting of equity data strategy and analysis with internal and external partners. Supports quality improvement for projects and programs. Supports reporting and data presentation for internal and external stakeholder.



  • Collaboration & Adaptability- Leads cross-functional projects and teams on complex business challenges. Interprets / addresses business challenges; develops best practices and oversees execution. Works with team and partners to address challenges that may impact community collaboration. Comfortable and willing to engage with community members and organizations. Demonstrates emphasis on relationship building / relational work, flexibility, and adaptability to change. Open to continuous learning and capable of taking a systems-learning view.





JOB QUALIFICATIONS



  • Bachelor's degree in a related field and 7+ years of work experience in a related job discipline OR Master's degree and 4+ years' experience
