In our statewide Corrections and Community Reentry Services, we provide specialized case management services to help people prepare to return to their families and communities after they've been incarcerated. We work both inside prisons and in communities to conduct screening and assessments for substance use disorders, place clients into treatment programs, and provide support with such activities as insurance enrollment, obtaining a state ID, and finding employment resources.


We are currently looking for full-time – Community Support Specialist

Starting at $39,000 – contingent upon experience, education, etc.


Division Overview: Permanent Supportive Housing (PSH) utilizes TASC's longstanding expertise in specialized case management to assist individuals in St. Clair and Madison counties in obtaining and maintaining affordable housing. PSH provides clients with the skills and resources necessary to thrive in their homes and communities.


Position Summary: TASC Community Support Specialist will assist clients in obtaining housing. The Community Support Specialist will assist clients to develop supportive stabilization plans and Assist in identifying long and short term goals. CSS will conduct housing assessments, assist with income applications, utility connections, rental applications and governmental resources. CSS will identify behavioral health, and other social determinates to health needs. TASC Community Support Specialist will provide referrals based on identified needs at the request of individuals enrolled into TASC services. PSS will link the client to additional case management services based on the client's personal willingness and needs.


Essential Duties:

  • Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources.
  • Will conduct eligibility assessments, in-person and virtually
  • Identify available housing options
  • Familiarize clients with their tenants' rights
  • Coach clients how to resolve landlord issues
  • Assist in determining client rental payments according to program procedures and guidelines
  • Conduct client outreach and assessments
  • Collect documents from the client or service providers at partner providers for assessment and reassessment
  • Create client housing stabilization plan that matches the needs of the client identified in the assessments
  • Ensure medical providers info is documented in the database and appropriate releases of information is on file Complete all supportive services referrals as necessary to provide and remove barriers addressing housing stabilization
  • Assist client in financial literacy capacity building through family budget training and assisting client in obtain a copy of their credit report
  • Assist in client move- in request for support
  • Assist client with utility reconnection process
  • Assist with housing start – up kit requests for support
  • Participate in the reconciliation of rental payments to landlord and client
Requirements:

Our Ideal Candidate:

  • High school diploma or GED
  • At least two years administrative experience
  • At least three years' experience in data entry, using the Internet, extensive user experience with computer hardware and software.
  • At least two years' experience with various software applications, including Microsoft Office, including Access.
  • Technical aptitude, ability to understand and follow technical instructions. Trained in various TASC computer systems.
  • Basic knowledge of database structures, queries and data maintenance.
  • Experience working with external partners, collecting, verifying and maintaining information.
  • Ability to make cold calls and represent the agency in a professional manner.
  • Keep informed of all policies, procedures and regulations that apply to this position.
