Founded over forty years ago as Lincoln Physicians, the Mass General Brigham Healthcare Center in Concord continues to honor its founders' legacy by delivering compassionate, dedicated, personal health care to families who live and work in the Concord, Massachusetts area. We are a member of the Mass General Brigham Community Physicians and a proud affiliate of Emerson Hospital.
We are thrilled to be providing primary care services at our new, state-of-the-art location at 80 Beharrell Street in West Concord, Massachusetts. As a Family Medicine office, we provide physical examinations, routine health screenings, and preventative care for all ages, from infants through adults.
The Opportunity
We are seeking a Full-time 40-hour RN to support our practice onsite Monday-Friday from 8:00am-4:30am with 1 remote day on either Tuesdays or Thursdays. The Family Medicine Registered Nurse (RN) provides professional nursing care to patients in a family medicine setting. The RN will work collaboratively with physicians, advanced practitioners, and other healthcare professionals to deliver patient-centered care, focusing on health promotion, disease prevention, and the management of acute and chronic conditions.
This role is 50% on the phones with patients and 50% patient facing!
Responsibilities Include:
• Triage clinical questions via phone calls.
• Provide clinical advice and education to patients based on assessment findings and established protocols.
• Provide appropriate care coordination for patients such as scheduling appointments, follow-up care and referrals to specialists and other healthcare services.
• Follow-up on emergency room visits.
• Collaborate with physicians, advanced practitioners, and other healthcare team members to ensure comprehensive patient care.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Provide direct patient care, including assessments, planning, implementation, and evaluation of care plans.
• Administer Injections, medications, treatments, and procedures as prescribed by physicians and advanced practitioners and assist in minor surgical procedures.
