Glade Run's Psychiatric Mobile Medication Program serves Butler County adults 18 years or older with a psychiatric diagnosis, a history of inpatient hospitalizations, and difficulties in their progress towards recovery. The program provides the education, support, and skill development needed for individuals to become independent and responsible for their medications.
Academic Requirements:
1. Licensed Practical Nurse (LPN); Registered Nurse (RN) preferred.
Emotional Requirements:
1. Must have emotional stability to perform essential job functions.
Other Requirements:
1. Minimum of one year of supervisory experience in the mental health or physical health profession preferred.
2. At least one year of experience working with seriously mentally ill patients.
3. Must possess verbal and written communication skills.
4. Valid driver’s license and acceptable driving record is required.
5. Ability to drive for work purposes including to client homes and to job sites throughout Butler County.
6. Must be able to adjust to job requirements.
7. Ability to implement agency policies and programming including but not limited to the following, Medication adherence, Medication
administration, Medication reconciliation.
8. Ability to make sound judgments regarding client care.
9. Must have acceptable PA State Police Clearance, PA Child Abuse History clearance, FBI clearance and Arrest/Conviction Report
and Certification form.
10. Must be able to identify with the agency Mission Statement and support the agency’s Core Values.
11. Must complete and pass agency Community Crisis Intervention training.
CORE COMPETENCIES
1. Collaborate with other service providers to ensure seamless transition of care from higher levels of care to the Psychiatric Mobile
Medication program and at discharge from PMM.
2. Collaborate with prescribers to obtain written scripts for individuals in the Psychiatric Mobile Medication program.
3. Communicate any medication related side effects, refills and/or compliance issues with the Prescribing physician.
4. Administer long acting injections for consumers with an active prescription in the Psychiatric Mobile Medication program.
5. Ensure medications are stored, administered, and disposed of in accordance with all applicable regulatory entities.
6. Performs medication counts: Daily for controlled substances, weekly for all other medications. Reports discrepancies immediately to
supervisor, completes all corresponding documentation.
7. Collaborate to ensure any necessary lab work is obtained prior to the distribution or administration of medications.
8. Collaborate with the pharmacy to obtain medications.
9. Ensure medication reconciliation occurs for any medications handled by Glade Run Lutheran Services.
10. May be required to perform services in clients’ homes.
SUPERVISORY COMPETENCIES
1. Responsible for providing oversight and monitoring of client medications; education regarding medications; linkage with appropriate
resources and supportive case management services.
2. Assists in the development and administration of the policies and procedures necessary for the operation of the Mobile Medication
Program.
3. Participates in the agency hiring process and supervision of the Mobile Medication Program staff.
4. Supervises mobile medication staff in the completion of direct service activities.
5. Lead, motivate, and coach others in a fast-paced environment.
6. Provide coaching and feedback to employees on performance related issues
7. Ensures financial integrity of the program via coordination of managed care authorizations, schedule of service delivery and staff
coverage.
8. Collaboration with stakeholders and other providers for linkage to appropriate resources and supports.
9. Participate in Quality Improvement activities to enhance positive consumer outcomes.
10. Schedules adequate staff coverage for the Mobile Medication Program.
11. Provides regularly scheduled group and/or individual supervision of mobile medication staff.
12. Provides oversight in the referral process; development of treatment plans, treatment reviews and other related documentation of
the mobile medication program.
13. Attends staff meetings, conferences and in-service training as required.
14. Conducts and documents treatment planning sessions and treatment reviews as a member of the interdisciplinary treatment team.
15. Maintains a small caseload of clients.
16. Assists in program referrals to include admissions and service coordination.
17. Collaborates with various program supervisors to ensure expeditious service delivery to clients.
18. Performs other related duties as may be required or requested.
19. Participates in the on-call schedule.
