Job Overview
The LVN, Clinical Support Partner position has the responsibility and authority to implement the education process for Registered Nurses, Licensed Vocational Nurses, Certified Nursing Assistants, Medical Assistants, and Personal Care Attendants. The LVN, Clinical Support Partner will conduct assessments, planning, implementation, and evaluation of competency assessments to achieve patient-centered care across all PACE and ACS locations based on the Center for Medicare and Medicaid Services (CMS) - PACE Regulations and the California Department of Healthcare Services (DCHS). This role involves collaborating with the PACE Clinical Nurse Informatics Specialist, various PACE departments, and PACE functional leaders to develop standardized materials for training purposes. In addition, this role is responsible for developing and implementing standardized, cost-effective, efficient training protocols, workflows, processes, systems, and key initiatives for the PACE and ACS clinical nursing departments to provide quality patient care.
Minimum Requirements
Graduation from an accredited Licensed Vocational Nurse School.
Current unrestrictive LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians.
Bachelor’s degree in Nursing, preferred.
Current California Driver’s License, required.
Current First Aid and BLS Certification, required.
Minimum of 1 year of experience working with frail elderly (Geriatric) population as required by CMS PACE regulations.
Minimum of 2 years of work experience in a PACE program is required.
Minimum of 2 years of hands-on training experience is required to develop training programs, procedures, and routines, preferably in healthcare organizations preferred.
Experience with developing Learning Management System training modules is preferred.
Prior experience with an Electronic Health/Medical System, required.

$31.00 - $36.00 hourly

The range is subject to change.
