In this essential position, you'll provide indirect services while collaborating closely with Nurse Care Managers to elevate patient care across our community. As a pivotal part of our team, you'll not only supervise but also empower your colleagues through training, support, and guidance.
What You'll Do:
Lead with Purpose: Oversee and evaluate nursing assessments, ensuring the highest standards of care for individuals served.
Empower Your Team: Train and mentor Nurse Care Managers, fostering an environment of continuous improvement and excellence.
Drive Quality Care: Utilize our systems to monitor service delivery, running reports and providing constructive feedback to enhance performance.
Educate and Inspire: Share your knowledge on medications, diagnoses, and health issues with nursing and clinical staff, ensuring everyone is equipped to provide exceptional care.
Collaborate for Success: Attend case reviews and rounds to make impactful treatment recommendations and support our prescribers and RNs.
Qualifications/Requirements
RN. license issued by Massachusetts Board of Registration required.
Care Management/Integrated care experience preferred.
Minimum of two years of nursing experience required, preferably community health/management and/or behavioral health and SUD.
Valid driver's license with a verified clean driving record and reliable vehicle available for all working hours required.
Demonstrates willingness and ability to work with client populations from diverse cultural and socioeconomic backgrounds.
