BASIC FUNCTION:  Under the supervision of the LVN charge nurse and/or the Director of Nursing (DON) at Residential Communities, responsible for providing resident care, which includes meal service, housekeeping, toileting, personal hygiene, activities of daily living, and medication assistance. Monitors health status and provides a safe environment for a residents.  Adheres to all Federal, State, and local regulations and corporate and facility Policies and Procedures. 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 


  • Ensures that all Residents are served in a courteous, efficient and effective manner.  Ensures that Resident needs and desires are met quickly and appropriately.

  • Follows established performance standards and duties according to nursing policies and procedures.  Assists in the administration of non-nursing and routine aspects of Resident’s living.

  • Identifies Resident concerns and reports them immediately to nurse/ supervisor.

  • Reports all unusual issues, incidents or changes in Resident’s condition to nurse/supervisor. Documents incidents and/or unusual situations according to established departmental procedures.

  • Identifies safety hazards and emergency situations.  Initiates corrective action as appropriate and reports situation to charge person/supervisor.

  • Creates and participates in group (s) and one-on-one activities in accordance with the activity calendar and as requested by the activities coordinator.

  • Demonstrates dependability through consistent compliance with scheduled work hours.

  • Maintains the confidentiality of all company procedures and information about participants, clients, or families.

  • Practices standard precautions and follows appropriate Infection Control procedures.

  • Participates in continuing education classes and any required staff and training meetings.

  • Documents the care and treatment provided to the Resident and the Resident’s response to care provided including completing required shift documentation.

  • Ensures care to all Residents assigned.  Assists Residents to dining room as needed, or makes meals available to Resident’s room.  Provides Residents toileting opportunities.  Assists with Resident baths, showers, or bed baths as needed.  Observes infection control procedures at all times.

  • Assumes responsibility for adhering to procedures on the use and control of equipment and supplies within the unit: including but not limited to dietary, housekeeping and activities.

  • Complies with all Federal, State, and local regulations.

  • Follows all procedures for admission, discharge, and transfer of Residents.

  • Understands and works well within a Resident centered environment and acts as a role model for compassion and understanding towards the Resident to all staff, guests and families. Involve residents in daily activities, maintaining records which indicate preferences and attendance.

  • Conforms to all Resident Rights regulations, promptly reports any observations or suspicions of Resident abuse or neglect to supervisor or other person of authority.

  • Works cooperatively with Residents, supervisors, co-workers, physicians, families, consultant personnel and ancillary service providers.

  • Performs other duties as may be assigned by supervisor or Management Staff.

  • Must meet all initial and annual in-service training requirements according to state regulations

MINIMUM QUALIFICATIONS:  Current valid First Aid and CPR cards.  Neat in appearance with good grooming habits.  Must be able to be tactful and courteous at all times with both Residents and employees.  Must be able to respond to Resident’s needs and expectations and have a desire to work with Dementia residents.  Must be computer literate sufficient to perform all tasks required of the Electronic Medical Records (EMR) System. Able to read, write, understand, and speak English.

EDUCATION AND EXPERIENCE:  A minimum of one-year experience working with the elderly in a skilled nursing or residential care facility preferred.  Dementia experience preferred
