Job Description:

The SUD Case Manager serves as the initial point of contact for referrals and access to resources and prevention services for individuals with Substance Use Disorder (SUD) for Volunteers of America Chesapeake and Carolina’s (VOACC) SPF-PFS Program. This role focuses on prevention efforts and relapse prevention, providing coordinated, individualized support to clients to address their specific needs and help them achieve their goals. The Case Manager will maintain a caseload of 20 clients and utilize a comprehensive approach to link clients with appropriate services and support.

Responsibilities:


  • Serve as the initial point of contact for referrals and access to resources and treatment for individuals with SUD.

  • Provide individualized case management services to clients, including assessment, goal-setting, and ongoing support.

  • Coordinate and facilitate referrals to services for therapy, groups, medication management, Medication-Assisted Treatment (MAT), and other Social Determinants of Health (SDOH) as needed.

  • Organize individual and small psychoeducation groups, both in-person and virtual, tailored to specific target audiences such as pregnant and parenting women, veterans, LGBTQ+ individuals, and youth and including mental health promotion services, in accordance with SPF-PFS Program Grant Requirements.

  • Utilize Motivational Interviewing techniques to support clients in their recovery journey and prevent relapse.

  • Conduct screenings and assessments, such as the Patient Health Questionnaire-9 (PHQ-9) and the Columbia-Suicide Severity Rating Scale (C-SSRS), as needed to provide appropriate referrals and support.

  • Maintain accurate and up-to-date client records, including progress notes and treatment plans.

  • Collaborate with internal and external stakeholders, including community organizations and treatment providers, to ensure comprehensive care for clients.

  • Maintain CPR/First Aid Certification and respond effectively in crisis situations.

  • Demonstrate cultural competency and sensitivity in working with diverse populations, including youth, LGBTQ+ individuals, pregnant women, military/veterans, and individuals in recovery.

  • Participate in ongoing training and professional development

Qualifications:


  • Must have a Bachelor's degree in a Human Services related field.

  • CSAC certification is preferred.

  • A minimum of one year of experience in case management, preferably in SUD, is required.

  • Experience working with diverse populations, including youth, LGBTQ+ individuals, pregnant women, military/veterans, and individuals in recovery.

  • Lived experience in recovery from substance use disorder is a plus.

  • Proficiency in Microsoft Word, Excel, and PowerPoint.

  • Excellent interpersonal, written, and oral communication skills.

  • Ability to use independent judgment and address conflicts to find resolutions.

  • Bilingual proficiency is a plus but not required.

  • Experience in Motivational Interviewing techniques is preferred.

  • CPR/First Aid Certification must be acquired and maintained once employed.
