Telehealth Physical Therapist (PRN) - Napa Post Acute
Napa Post Acute is looking for a skilled and compassionate Telehealth Physical Therapist to join our team on a PRN basis. Position: Telehealth Physical Therapist (PRN)Location: Napa Post Acute (Remote/Telehealth)Employment Type: PRN (as needed)
Responsibilities:
Conduct virtual physical therapy evaluations and treatments via telehealth platforms
Develop individualized treatment plans to improve patients' mobility, function, and quality of life
Provide guidance and exercises that can be performed at home
Track and document patient progress using electronic health records (EHR)
Collaborate with an interdisciplinary team to ensure continuity of care
Qualifications:
Valid Physical Therapist license in the state of California
Experience in telehealth or virtual care delivery is preferred
Strong clinical skills and the ability to work independently
Excellent communication and tech-savvy skills
Ability to manage flexible, PRN hours
