Join our team of competent and compassionate professionals as the Program Director in the Program of Assertive Community Treatment at Behavioral Health Network!

This position splits time between Holyoke, MA and Springfield, MA.



GENERAL SUMMARY


The Program Director oversees the day-to-day operations of the PACT team. The Program Director coordinates and oversees the clinical care for individuals served at PACT. The Program Director manages the coordination of care with other agencies and licensing bodies. The Program Director directs the day-to-day clinical operations of the PACT team including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours; leads the daily organizational staff meetings and treatment planning meetings; continuously evaluates the status of clients and does appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs.



ESSENTIAL JOB FUNCTIONS:


  • Maintains communication with funding sources or other contacts, as appropriate.

  • Ensures that the program adheres to regulations and requirements of licensing bodies and funding sources.

  • Serves as primary liaison with other BHN programs.

  • Directs and coordinates the client admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Schedules the admission interview; develops and coordinates the initial assessment and initial treatment plan; assigns the most appropriate staff to the individual treatment team (ITT); and provides clinical supervision of the development of the comprehensive assessment and the treatment plan for each client.

  • Carries out directions of senior management.

  • Conducts regularly scheduled individual and team supervision in accordance with BHN policies and documents individual and team supervision.

  • Ensures all staff are following MAP Policies and Procedures.

  • Assigns the most appropriate staff to the individual treatment team.

  • Provides clinical supervision of the development of the comprehensive assessment and the IAP for each individual.

  • Serves as part of 24-7 on-call rotation.

  • Ensures all program documentation is complete and meets PACT, agency, and third-party standards. Documentation includes assessments, treatment plans, contact notes, risk plans and crisis plans.

  • Provides comprehensive Intake Evaluations with clinical formulations and
    recommendations based on sound clinical principles and judgment.

  • Includes all pertinent family members, community, and residential providers in treatment, as appropriate.

  • Assures assigned clients make positive change on the outcome measure used by the PACT Model.

  • Provides comprehensive risk/crisis evaluations and takes appropriate actions to keep clients and others safe.

  • Maintains flexibility in providing services in settings such as client homes, hospitals, and other community settings.

  • Maintains professional licensure and completes the required amount of clinical education hours.

  • Encourages independence, competency, and decision-making.

  • Initiates, supports, and ensures quality of life through empowerment, safety awareness and respect and dignity.





OTHER DUTIES AND RESPONSIBILITIES

  • Maintains PACT specific Policies & Procedures.

  • Ensures PACT team are trained in PACT Model.

  • Works weekends, second shifts, and holidays as needed.

  • Completes progress notes for all individuals by the end of each shift.

  • Completes program specific forms and required, time-sensitive documents such as Incident Reports and Medication Occurrence Reports in a timely manner.

  • Attains competence in motivational interviewing and utilizes this approach when working with ambivalence.

  • Ensures Smartsheet is completed each shift and that change of shift occurs each shift with oncoming staff.

  • Oversees BHN Vehicles and maintenance of properties.

  • Completes health and medication-related documents as individually indicated and in accordance with MAP regulations.

  • Provides on-going information, periodic reviews, and documentation to support treatment and Treatment Plan goals and objectives.

  • Completes initial assessments, treatment plans, treatment plan reviews, financial management plans, and assessments to assigned caseload as required by supervisor.

  • Other duties as assigned.




PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

  • Master's Degree in Social Work, Psychology, or Mental Health Counseling required.

  • Clinical licensure required (LICSW Preferred).

  • Previous supervisory experience required.

  • Minimum of 5 years in human services preferred.

  • Must be committed to empowerment and growth, human rights, and community integration of individuals served.

  • Demonstrates willingness and ability to treat challenging and diverse client populations, including a forensic population.

  • Demonstrates cultural competence in supporting clients with a variety of different cultural backgrounds.

  • Demonstrates working knowledge of community resources, and how to help clients access them.

  • Demonstrates willingness to work with individuals who are forensically involved within the Sequential Intercept model.

  • Must have valid driver's license and a reliable car available for all working hours to transport individuals served required.
