Position Purpose:





  • The Care Management Support Coordinator II supports administrative care management activities by performing outreach, answering inbound calls, and scheduling services. This role serves as a point of contact for members, providers, and staff, resolving issues and documenting member records in accordance with state and regulatory guidelines.




Day-to-Day Responsibilities:





  • Conduct outreach to members via phone to assist with care plan next steps, community resources, and scheduling concerns.

  • Answer inbound calls from members and providers, addressing inquiries and providing information about care plan procedures.

  • Schedule services and support member onboarding by sending welcome letters and educational materials.

  • Document and maintain non-clinical member records, ensuring compliance with policies and regulatory requirements.

  • Connect members to health plan and community resources to enhance customer care.

  • Provide assistance with social determinants of health (SDOH) needs and make appropriate referrals.

  • Collaborate with team members and maintain clear communication with all stakeholders.




Performance Expectations/Metrics:





  • Timely and accurate documentation of member interactions.

  • Responsiveness to member and provider inquiries.

  • Quality of outreach and follow-up activities.




Required Skills/Experience:





  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).

  • Strong spoken communication skills.

  • Accurate typing skills.




Preferred Skills/Experience:





  • Prior call center experience.

  • Experience in a managed health plan or medical facility.




Education Requirement:





  • High School diploma or GED required.
