Overview

  • Advocates for patients and families, screens for abuse, suicidal ideation, and medical neglect and makes appropriate referrals; screens patients’ and families’ ability to cope with major life changes; provides support to patients and families referring to Behavioral Health Consultant as appropriate; provides family with information regarding patient care issues; works with patients and families on guardianship and conservatorship; facilitates in the coordination of follow up care; participates in patient and family’s health care education; assists with housing and transportation needs as necessary; performs drug and alcohol screening and refers for appointment follow up.

  • Participates in department meetings; clinic committees, participates in workshops, seminars and training sessions to maintain professional competency; assists in staff and student education; visits community facilities, programs or services to acquire pertinent resource information. May participate in Behavioral Health program development and growth, such as clinic workflow implementation, scheduling, and developing clinic processes. May lead patient groups as needed. May assist in administrative duties that require attention to detail.

  • Maintains accurate and timely documentation of patient care activities an efficent manner per departmental policies. Complete all OHSU/Joint Commission mandatory education requirements and activities within established time lines. Yearly competencies as identified by department.



Responsibilities

  • A Bachelor’s degree, OR three more years of experience which shows:

    • a) knowledge of case management and strong customer services skills;

    • b) knowledge of social system navigation;

    • c) knowledge of medical terminology; and

    • d) knowledge of implications of the effect of illness, injury and/or disability on patients and families.

  • Two years of social service work experience providing collaborative case management to clients in a team based approach.

  • Demonstrated ability to network with local community partners.

  • Intermediate level skills in Microsoft Word.

  • Demonstrated ability to work with a variety of diverse customers in difficult situations.

  • Skill in developing and maintaining professional relationships quickly.

  • Ability to prioritize work demands and manage time in a fast paced environment.

  • Ability to interpret and follow policies and procedures.

  • Ability to perform the job duties with or without accommodation.


Qualifications



  • Bachelor’s Degree in Social Work (BSW).

  • Experience within a medical service delivery system or health plan (Medicare, Medicaid).
  • Experience working with an electronic health record; EPIC platform preferred.

  • Experience with Microsoft Excel.

  • Oregon Registered Baccalaureate Social Worker Certificate/License.
