Summary:
Provides pharmaceutical care for patients in a specialty clinical area. Supports professional development of clinical pharmacists, students and/or residents.
Responsibilities:
Provides Clinical Pharmacy services which may include but is not limited to: renal/hepatic dosage adjustment, pharmacokinetic dosing, anticoagulation monitoring, total parenteral nutrition, antimicrobial stewardship.Reviews, interprets and evaluates medication orders, labs and other clinically relevant information to ensure safe and optimal therapeutic outcomes and documents interventions appropriately.Manages and recommends drug therapy regimens as a part of an interdisciplinary team to provide patient and family centered care.Provides drug information to patients and healthcare providers including medical, nursing, and pharmacy staff.Supervises and delegates tasks to technicians and other support staff to ensure timely prescription/order verification, preparation, and distribution of medications, compounding and/or supplies and coordinates workflow as needed.

Accredited Program: Pharmacy (Required)Pharmacist License (PS LICENSE) - State of Florida (FL)
Additional Job Information:
Complexity of Work: Requires critical thinking skills, effective communication skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Required Work Experience: PGY2 residency or PGY1 residency/fellowship plus one (1) year of clinical experience in a specialty area or PGY1 residency/fellowship plus active board certification. Alternatively, active board certification plus three (3) years of clinical experience in a specialty area. Other Information: Additional Education Info: Doctor of Pharmacy or BS in pharmacy from an accredited School of Pharmacy
