JOB PURPOSE:

The Director of Complex Care and Social Needs (CCSN) is responsible for the management and oversight for CCSN Programs. The CCSN Programs consist of outreach initiatives designed to address a variety of Social Determinant of Health needs including access to health insurance, preventative health care, linkage to substance use recovery and behavioral health services, assisting returning citizens for workforce development and the coordination of health/social/economic needs for members of the community. The CCSN Director has responsibility for administrative and program oversight related to budget management, personnel, quality reporting, and outcomes as set by the program metrics and the program partner business requirements. S/he is responsible to hire, train, manage, and effectively supervise staff ensuring adherence to policies and procedures as it relates to program fidelity and practices. Director provides ongoing communication and collaborative engagement activities with funders/partners.

ESSENTIAL DUTIES:

The primary responsibilities of this position are:


  • Provide leadership, general oversight for daily operations of the CCSN department (i.e., budget, program development, partnership building, development of SOP’s, hiring, supervision and training)

  • Help develop new initiative proposals, progress reports, requests for proposal responses, grant applications and other documents and presentations.

  • Complete timely and accurate programmatic reporting required by funders and HCAM leadership.


  • Work closely with both internal and external agency partners, administrators, and staff to develop workflows related to health insurance enrollment, outreach, coordination of care and referral networks.

  • Conduct and assist with continuous quality improvement activities related to accurate completion of core work activities (enrollment, care coordination, connection to resources).

  • Attend relevant trainings, workshops, conferences, and networking opportunities, in line with the role and responsibilities of the position, and at supervisor’s discretion; and

  • Other duties as assigned 

QUALIFICATIONS:

Education, Experience, Certifications and Licensures:


  • Master’s Degree in Health Policy, Health Administration, Public Health, or another related field of study.

KNOWLEDGE, SKILLS & ABILITIES


  • At least three years of program management/supervisory experience required.

  • Experience developing and maintaining relationships with a variety of external partners.

  • Thorough knowledge of the Accountable Care Act, Maryland Medicaid/HealthChoice System, the Maryland All-Payer model and other evidence-based service delivery models.

  • Exceptional interpersonal & communication skills

  • Excellent time management, goal setting, and organizational skills

  • Professional manner and appearance

  • Proficient in all Microsoft office products; EMR experience preferred

  • Strong business acumen and relationship building skills among diverse personalities, building trust and credibility with senior level management.

  • Demonstrated ability to effectively manage a diverse team in a fluid, demanding environment.

  • Excellent project management skills.
