Shift: Shift 1 - hybrid role after 6 months of on-site training. Staff can then work remotely 2- 3 days a week. Monday through Friday 8-hour days. hours can vary from start time. this role supports all phases of the pre transplant kidney coordinator work from intake/referral, to clinic, to waitlist management.

Job Summary:

The Kidney Transplant Coordinator will act as a liaison between families, hospital staff and patients who are pre and/or post Kidney transplant. Responsibilities include participating in the clinical management of the patient with all necessary disciplines to coordinate patient services and education across the continuum of care.

EXPERIENCE DESCRIPTION:

Minimum of 3 years of acute care or dialysis experience is required.
Experience relevant to Nephrology/Transplant subspecialties is preferred.

EDUCATION DESCRIPTION:

Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required.
Bachelor's degree in Nursing is preferred.

Hands on patient experience in an intensive care setting. High degree of organizational skills and excellent communication skills; ability to work independently and take programmatic initiative. Knowledge of data systems, outcomes data and other clinical performance data.

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.
