Key Role: Oversee the MDS process to ensure accurate assessments and optimal reimbursement.

Key Responsibilities:
Complete and coordinate the Minimum Data Set (MDS) assessments for residents in compliance with federal and state guidelines.
Work closely with nursing, therapy, and other interdisciplinary teams to ensure accurate and timely assessments.
Ensure accurate documentation to support reimbursement and compliance with regulatory standards.
Conduct care planning meetings and collaborate with staff to ensure individualized care plans.
Maintain up-to-date knowledge of MDS regulations and Medicare/Medicaid reimbursement guidelines.
Qualifications:
Valid LVN license in the state of California.
Previous experience in MDS coordination in a skilled nursing or long-term care setting.
Strong attention to detail, organizational, and communication skills.
Knowledge of MDS 3.0 and understanding of Medicare/Medicaid regulations.
Ability to work effectively under pressure and meet deadlines.
