Youth & Family Services (YFS) help parents and young people who are striving to become healthy and restore their relationships. This division works in partnership with human services, child welfare systems, juvenile courts, and community-based service providers so that together we can help individuals and families obtain the services and skills they need to live in positive, healthy ways.


We are currently looking for full-time – Case Manager

Starting at $41,600k – contingent upon experience, education, etc.


ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

  • Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources.
  • Assess, evaluate, facilitate, and monitor individuals' progress in relationship to established goals and objectives
  • Complete intake packets for program enrollment and participation
  • Provides referrals and crisis intervention for the individuals' identified needs
  • Interview individuals, review records, and confer with other professionals to review individual progress, and to determine their suitability for program participation
  • Knowledge of DSM V and ASAM criteria
  • Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services.
  • Identify and refer individuals with behavioral health needs
  • Complete and submit internal and external program reports
  • Complete and maintain all required individual and program related documentation which includes, documenting the results of screenings, enrollments, service plans, reports, progress notes, discharge summaries and other individual-related activities including accurate reporting of services
  • Monitor all individuals determined by individual progress towards service goals
  • Monitor TASC database system and contractual/external database systems to ensure Individual enrollment
  • Participate in community meetings, trainings and projects including multi-disciplinary staffing, vocational and educational-programming, institutional counseling, correctional-industries, employers and other meetings assigned.
  • Develops effective working relationships with the internal and external partners
  • Participate in meetings to adjust, update or revise individual service plan
  • Obtain and clinically analyze progress reports from internal and external partners to ensure individuals ‘progress is on target with the benchmarks outlined in their service plan
Requirements:

Our Ideal candidate:

  • Bachelor's degree preferred and/or three years lived or professional experience.
  • Ability to obtain CADC certification through the State Certification Board (IAODAPCA) within the first two years in the field
  • Demonstrated proficiency in the Microsoft Office suite
  • Demonstrated experience in group facilitation
  • Certain programs may require a higher level of education, certification and/or licensures based on contractual obligations

The agency currently offers hybrid work schedules that combines in-office and remote work. Employees are required to report to the work site three days per week with the option to work up to two days remotely.
